What nursing interventions are appropriate for Mrs. J. at the time of her admission? Drug therapy is started for Mrs. J. to control her symptoms. What is the rationale for the administration of each of the following medications?

Mrs. J. is a 63-year-old woman who has a history of hypertension, chronic heart failure, and sleep apnea. She has been smoking two packs of cigarettes a day for 40 years and has refused to quit.

Use the following Case Scenario, Subjective Data, and Objective Data to answer the Critical Thinking Questions.

Case Scenario

Mrs. J. is a 63-year-old woman who has a history of hypertension, chronic heart failure, and sleep apnea. She has been smoking two packs of cigarettes a day for 40

years and has refused to quit. Three days ago, she had an onset of flu with fever, pharyngitis, and malaise. She has not taken her antihypertensive medications or her

medications to control her heart failure for 4 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure.

Subjective Data
1.Is very anxious and asks whether she is going to die.
2.Denies pain but says she feels like she cannot get enough air.
3.Says her heart feels like it is “running away.”
4.Reports that she is so exhausted she cannot eat or drink by herself.

Objective Data
1.Height 175 cm; Weight 95.5 kg
2.Vital signs: T 37.6 C, HR 118 and irregular, RR 34, BP 90/58
3.Cardiovascular: Distant S1, S2, S3, S4 present; PMI at sixth ICS and faint; all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac

monitoring indicates a ventricular rate of 132 and atrial fibrillation
4.Respiratory: Pulmonary crackles; decreased breath sounds right lower lobe; coughing frothy blood-tinged sputum; SpO2 82%
5.Gastrointestinal: BS present: hepatomegaly 4 cm below costal margin

Critical Thinking Questions

What nursing interventions are appropriate for Mrs. J. at the time of her admission? Drug therapy is started for Mrs. J. to control her symptoms. What is the rationale

for the administration of each of the following medications?
1.IV furosemide (Lasix)
2.Enalapril (Vasotec)
3.Metoprolol (Lopressor)
4.IV morphine sulphate (Morphine)

Describe four cardiovascular conditions that may lead to heart failure and what can be done in the form of medical/nursing interventions to prevent the development of

heart failure in each condition.

Taking into consideration the fact that most mature adults take at least six prescription medications, discuss four nursing interventions that can help prevent

problems caused by multiple drug interactions in older patients. Provide rationale for each of the interventions you recommend.

Jessica Williamson is a 28 year old women with a past history of epilepsy. Two weeks ago
Jessica was out socially, drinking alcohol at a dance club. Jessica experienced a seizure whilst
on the dance floor. Upon arrival at the local Emergency Department, paramedic staff reported that she had experienced a full tonic-clonic seizure that lasted for three minutes, during which she lost consciousness. Jessica was assessed and admitted. During her hospitalization Jessica was stabilized and discharged from hospital 1 week after the episode. Her discharge medications included an anticonvulsant.
On discharge Jessica was asked her GP for follow-up.
Medical History
Jessica was diagnosed with epilepsy in late adolescence. Initially the seizures were very difficult to control using anticonvulsants but her condition stabilised 5

years ago. Three months ago
she weaned herself off her anticonvulsants as she wanted to fall pregnant. During her hospitalization she was recommenced on her anticonvulsant.
Four years ago Jessica was diagnosed with Type 2 Diabetes Mellitus. Jessica was taught to
control her blood glucose levels using diet and oral hypoglycaemics, this is now decreasingly effective. Her most recent observations include:
•BP: 150/90
•P: 115 regular
•Height: 158 cms
•Weight: 122 kgs
•BGL 12.9
Her current medications are:
•Epilim1 gm BD
•Metformin 850mg BD
Social History
Jessica lives in a two-storey house with her current partner of six years, Martin. Jessica’s parents live overseas where her mother has been posted for work. Jessica

is an only child. She works as a
real estate agent selling homes and covers a large geographic area involving long and irregular work hours. This can involve her driving long distances and for long

periods of time. Jessica enjoys her work and the challenges of being independent at work and at home.
Despite being advised by her GP that she must not drive after suffering this recent seizure, Jessica continues to do so.
Current Issue
Following the follow-up appointment with the GP, the GP was concerned about Jessica’s
seemingly poor acceptance of her medical diagnoses and her compliance with the treatment regime for both her diabetes and epilepsy. Jessica indicated she did not need

any more prescriptions for her anticonvulsants at the moment.
Jessica is not particularly careful about monitoring her blood sugar levels. She says she often forgets, and tends to check when she “feels fuzzy”.

As this is your first visit to Jessica in her home, justify and discuss the immediate plan of care for her.
Assessment Focus
You are the Registered Nurse from the Community Nursing Service.
You will be undertaking an initial visit to Jessica’s home.
Prioritise and provide a rationale for the activities you will undertake as part of this initial visit
and identify two (2) priorities for your follow-up visit.

The purpose of this essay is for students to present a comprehensive discussion linking the health issues identified through a client assessment to the impact they

have on the client
and their level of function in the community setting. It requires the student to prioritise the health issues and to show analysis of the complexity of care provision

demonstrating critical reflection on practice.

Write a paper of 500-750 words (not including the title page and reference page) on your proposed problem description for your EBP project. The paper should address the following: Describe the background of the problem. Tell the story of the issue and why it deserves attention.

Nurses Improving Patient Satisfaction through meaningful hourly patient rounds

Nurses Improving Patient Satisfaction through meaningful hourly patient rounds

Order Description

Part 1 ( 1 page )
Before making a case for an evidence-based project, it is essential to understand the culture of the organization in order to begin assessing its readiness for EBP implementation. Complete the “Organizational Culture and Readiness for System-Wide Integration of Evidence-Based Practice Survey,” located in the textbook appendix. Develop an analysis of 250 words from the results, addressing your organization’s readiness level, possible project barriers and facilitators, as well as how to integrate clinical inquiry. Make sure to include the rationale for the survey categories scores that were significantly high and low, incorporating details and/or examples. Also explain how to integrate clinical inquiry into the organization, providing strategies that strengthen the organizations weaker areas. Submit a rough draft of the survey results with your narrative analysis. However, a final draft of the survey results should be placed in the appendices for the final paper. Prepare this assignment according to the APA guidelines . An abstract is not required. This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion. Upon receiving feedback from the instructor, revise “Section A: Organizational Culture and Readiness Assessment” for your final paper submission. This will be a continuous process throughout the course for each section. Rubric Describe the results from the Organizational Culture and Readiness for System-Wide Integration of Evidence Practice Survey addressing the readiness level of your organization. Provide an analysis of any possible project barriers and facilitators, and describe how to integrate clinical inquiry into your organization. Detailed information in relation to the survey categories that scored high and low on the survey is provided. The rationale for the scores, including details and/or examples, is provided. The major project barriers and facilitators are thoughtfully analyzed and evaluated. Warranted conclusions are drawn. An informed position on how to integrate clinical inquiry into the organization is developed and explained, providing strategies which align to the weaker areas of the organization. Clarity and specificity of comprehension are demonstrated, and all relevant information is synthesized. Coverage extends beyond what is needed to support subject matter. Writer is clearly in command of standard, written, academic English.
Part 2 ( 2 page)
Write a paper of 500-750 words (not including the title page and reference page) on your proposed problem description for your EBP project. The paper should address the following: Describe the background of the problem. Tell the story of the issue and why it deserves attention. Identify the stakeholders/change agents. Who, or what organizations, are concerned, may benefit from, or are affected by this proposal. List the interested parties, patients, students, agencies, Joint Commission, etc. Use the feedback from the Topic 2 main forum post and refine your PICOT question. Make sure that the question fits with your graduate degree specialization. State the purpose and project objectives in specific, realistic, and measurable terms. The objective should address what is to be gained. This is a restatement of the question, providing focus. Measurements need to be taken before and after the evidence-based practice is introduced to identify the expected changes. 5) Provide supportive rationale that the problem or issue is an important one for nursing to resolve using relevant professional literature sources. Develop an initial reference list to assure that there is adequate literature to support your evidence-based practice project. Follow the “Steps to an Efficient Search to Answer a Clinical Question” box in chapter 3 of the textbook. Use “NUR-699 Search Method Example” to assist you. 7) The majority of references should be research articles. However, national sources such as Centers for Disease Control and Prevention (CDC), National Center for Health Statistics (NCHS), Department of Health and Human Resources (HHS), or the Agency for Healthcare Research and Quality (AHRQ) and others may be used when you are gathering statistics to provide the rationale for the problem. Once you get into the literature, you may find there is very little research to support your topic and you will have to start all over again. Remember, in order for this to be an evidence-based project, you must have enough evidence to introduce this as a practice change. If you find that you do not have enough supporting evidence to change a practice, then further research would need to be conducted. Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required. This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion. You are required to submit this assignment to Turnitin. Please refer to the directions in the Student Success Center. Upon receiving feedback from the instructor, refine “Section B: Problem Description” for your final submission. This will be a continuous process Describe the background of the problem. Identify the stakeholders/change agents and list the interested parties. Provide the PICOT question. State the purpose and project objectives in specific, realistic, and measurable terms. Develop an initial reference list throughout the course for each section. —————————————————————————————————–

Nurses improving patient satisfaction through meaningful hourly rounds using AIDET 1.
Hourly rounding is a simple and cost-effective intervention to improve patient perception of
Evidence-based research indicated implementation of hourly rounding would increase pt. satisfaction, decrease fall rates, decrease skin breakdown rates, and increase staff satisfaction. All patient care staff in clinical areas was educated utilizing the Studer Group Hourly Rounding Module. http://www.mc.vanderbilt.edu/root/pdfs/nursing/hourly_rounding_supplement-studer_group.pdf
Information hcaps, Studer customer satisfaction ect.
Hourly rounds—intentionally checking on patients at regular intervals—continues to be debated in nursing circles. Often, registered nurses make rounds on even hours and support staff make rounds on odd hours from 6 AM to 10 PM (and every 2 hours from 10 PM to 6 AM). While making rounds, staff engage patients by checking on the “4 P’s”: pain, positioning, potty (elimination), and proximity of personal items. Patients are told that staff will check on them frequently, so hourly rounds help manage patients’ expectations. Patients become less anxious about getting their needs met as they learn to trust the process of hourly rounds.
Attending to patients’ comfort, safety, and environmental needs may also prevent adverse events like falls, pressure ulcers, or unrelieved pain; and contribute to patients’ satisfaction with nursing care. Proponents also attest that hourly rounds organize work flow, offering efficiencies by giving nurses time back as they proactively (rather than reactively) anticipate and attend to patients’ needs. In this review I discuss available evidence about the effects of hourly rounds on clinical outcomes in inpatient settings.

Profile Process Planning Sheet: Analyzing the Rhetorical Situation
Week 1 Assignment
Please answer the following questions in complete sentences and paragraph format. Although this is not a formal essay assignment, please note that proper spelling,

grammar, and sentence structure are required. This week’s lecture and chapters 2 and 3 in your text will help you work through some of the terms within the rhetorical

situation.
Defining Topic, Angle, Purpose
1.    What is your chosen topic? (For example: a profile of Barb Collins, a volunteer at the local homeless shelter; a profile of Shipwreck Days, a citywide flea

market and festival; or a profile of Burdick’s Grocery, the oldest family-owned business in town.)
A profile of Joelle Smart, a hairdresser with a new salon location in the basement of her home .
2.    What personal angle can you present on this topic? (Consider what is new about the topic or what unique perspective can be shared about this topic.)
Smart opened a salon on Main Street in the Westside neighborhood of Springfield in 1986. Recently, due to a boom in the real estate market by young professionals, the

landlord of the salon has nearly doubled Smart’s rent. Many new chain stores, restaurants, and coffee shops are lining up to pay the increased rent, but Smart can no

longer afford to stay in business on Main Street. In an attempt to keep her business running and continue to provide services for her clients, she is now operating out

of the basement of her home .
3.    What is your purpose for writing? (What do you hope to accomplish? Why are you writing?)
The purpose is to describe Smart’s struggle to stay in business due to rent increases on Main Street as a larger indication of the negative results that have come from

the neighborhood’s growth .
Analyzing Your Readers
You will be writing your Profile essay to your local community. Imagine you might submit the Profile to your local newspaper or have it shared in a community

newsletter; the readers of those publications make up your target audience. In two to three paragraphs, define your local community and describe what makes it unique.

What are the needs, expectations, motivations, backgrounds, demographics, and experiences of community members? How will those influences impact your word choice,

level of formality, and overall message?
The Westside community is unique in that it’s an old neighborhood with a great deal of history. As many of the older, longtime residents are dying, moving into nursing

homes, or downsizing in other areas of the city, young professionals have started moving in. When these college-educated men and women started to buy real estate in

the area, the commercial district also boomed. Over the last 5 years as the real estate boom has only continued to rise, many longtime retailers can’t afford the rent

increases and are being forced out in exchange for new businesses.
Springfield’s older residents have built a relationship with the older retailers in town and are committed to giving them their business. They are not happy about the

chain coffee shops or other franchises moving in. Interestingly the independent nature of the commercial district is what originally attracted many of the younger

population, yet they are giving their business to the new, higher-end retailers that are replacing many of the older shops.
The community is represented by new shop owners who are thriving and old retailers that are barely hanging on or are forced to close or relocate. Their customers are

also a mix of new and longtime residents, but the new, young professionals are quickly becoming the majority. They are college-educated with young children, and most

are married . My tone will need to be formal because this is primarily a well-educated group. It will not be appropriate to use humor or sarcasm. I should approach the

audience with respect and understanding, yet be sincere in expressing concerns. It will be important to really describe Joelle with details and descriptions so that

the readers feel as if they know her and care about her .
Reflecting on the Role of the Writer
Whether you realize it or not, your personal relationship to the subject plays a key role in shaping the Profile essay. Remember, your personal brand is shaped, in

many ways, by your community influences. Just as you make choices in terms of how you present your brand through writing, personal choices will also impact how you

present the subject of your Profile. In two to three paragraphs, discuss your personal relationship to the subject. How have personal impacts, needs, motivations, and

relationships shaped your topic, angle, and purpose?
Joelle Smart has been my hairdresser for nearly 15 years as I am one of the older residents of my neighborhood. She and many of the other longtime retailers have

become close friends, and I have always valued supporting local businesses. It’s hard to watch these friends struggle to pay their rents and stay in business. However,

in all honesty, I’ve enjoyed watching my neighborhood’s transition over the years. The energy of the young professionals has brought more culture and events to the

area, and our property values have increased with the surge in the real estate market. Still, I hate to see high-end retailers replace family-owned businesses that

have been a part of the community for years.
I will continue to be a client of Joelle’s in her basement salon, but I am concerned that her clientele will decline. Many of the young residents would prefer to visit

a salon right on Main Street and they will probably find her basement location less than hip. Without the Main Street location, it will be hard for Joelle to attract

new clientele. I’d like the community to realize the struggle that Joelle and other retailers are facing, and I hope to encourage them to continue to support her

business and the other older businesses in town. I’d also like them to realize the larger impact of so many new franchises coming into the area and warn them that we

are losing our independent roots by supporting these businesses .

newsletter; the readers of those publications make up your target audience. In two to three paragraphs, define your local community and describe what makes it unique.

What are the needs, expectations, motivations, backgrounds, demographics, and experiences of community members? How will those influences impact your word choice,

level of formality, and overall message?

Reflecting on the Role of Writer
Whether you realize it or not, your personal relationship to the subject plays a key role in shaping the Profile essay. Remember, your personal brand is shaped, in

many ways, by your community influences. Just as you make choices in terms of how you present your brand through writing, personal choices will also impact how you

present the subject of your Profile. In two to three paragraphs, discuss your personal relationship to the subject. How have personal impacts, needs, motivations, and

relationships shaped your topic, angle, and purpose?
