Purpose

To provide consisted symptom based triage and maintain processes in timely manner during software downtime.
This procedure provides nurse triage with general guidelines for handling calls during scheduled or unscheduled downtime.

Standard

1.    In the event of Epic Downtime, scheduled or unscheduled, nurses triage is expected to immediately inform his/her manager of the event.
2.    The Nurse Triage manager carries responsibility of conforming the downtime with IT support services and communicating back to the team.
3.    There two types of Epic scheduled and unscheduled downtime, Shadow Read only and shadow Read Write.
4.    Calls are triaged using Schmitt Thompson protocols, which are maintain in hard copy and are located on carts behind nurse triage desk.
5.    In the event of Shadow read only
Nursing triage may not triage symptoms based calls without protocols.
There are three forms are used by nurse triage during downtime scheduled or unscheduled of shadow ready only and they kept in the business continuity and downtime

folder and in each desktop of nurse triage computer.

5.1    Downtime Nurse Triage Call Log which contains the call time, name of caller, and reason for call.
5.2    Downtime Nurse Triage Patient Encounter Form which contains caller demographic data and the nursing assessment questionnaire.
5.3    Downtime Nurse Triage Call Back Form which contains information for follow-up on calls; reason, phone number, and message.
After Epic downtime recovered all downtime forms scanned into Epic.

6.    In the event of shadow Read Write: nurse triage caregivers can document normally into Epic and data will be synced to the live environment at the end of the

downtime.

Oversight and Responsibility
1.    The Contact Center manager, Nursing Quality and Clinical Nursing is responsible for the oversight of this policy.
2.    The Contact Center Lead Triage Nurse is responsible for updating the Business Continuity and Downtime folder.
3.    All Contact Center Nurse Triage staff RNs are responsible for implementing this policy within the contact Center.

Definitions

1.    Downtime:  Time during which the electronic information system is unavailable.

2.    Scheduled Downtime:  Planned system outage for maintenance or upgrade, which is scheduled and communicated in advance.

3.    Unscheduled Downtime:  Unforeseen outage due to a system failure.

4.    Shadow Read Only (SRO) environment: Back-up read only snapshot of Epic that allows nurse triage to review Epic but all documentation must be done on paper.

5.    Shadow Read Write (SRW) environment: A back-up snapshot of live environment will allows caregivers to document normally into Epic, and later at the end of

downtime data will be synced into Epic.

femoral artery sheath was removed in theatre, the site compressed and an opsite dressing applied  to the site on his left inguinal region.  He is currently on half

hourly circulation observations.  He has an IV of 0.9% Sodium Chloride (1L) / 24 hourly infusing.  His initial return to ward observations were HR 80/min, RR 18/min,

BP 120/80, Temp 36.9°C.  When you perform your next set of observations 30 minutes later Michael’s observations are HR 110/min, RR 20/min, BP 100/70 and Temp 37° C.

He reports that he has pain in his groin and has a pain score of 4/10.  He also says “I really want to pee but I can’t while I’m lying down.”  His feet and hands feel

pale, clammy and cool. You lift up the sheet to inspect the catheter site and there is a large pool of fresh blood all over the sheet under Michael and a visible

leakage from the site.

For this assignment, you will be further defining your Phenomenon of Interest within a theoretical and ethical framework. The same Phenomenon of Interest (POI)

identified in your first written paper should also be used for this assignment. You will begin this paper by providing a concise description of your POI, this

Next, you will discuss the Fawcett’s Meta-paradigm of Nursing and relate the paradigm directly to your POI. How do the four concepts within the meta-paradigm relate to

your POI? Is one more important? Do all four have the same level of importance? You should support this section with peer reviewed references as appropriate.

Specifically identify the components of the meta-paradigm within your discussion. Be sure to reference the meta-paradigm appropriately.

Then, you will select both a Grand nursing theory as well as a Middle range nursing theory. Take time to review several examples of each type of theory as the selected

theories need to “fit” your POI…and work well together.

Grand theory discussion: identify and discuss the inter-related concepts from your selected theory. What aspects of the nursing meta-paradigm are addressed by your

grand theory? Then provide information about how you will view your POI through the lens of the grand theory. How does the theory guide your assessment of the POI? How

does the theoretical framework chosen categorize or define your POI? How does the theoretical framework effect your perception of the POI?

Middle range theory discussion: identify and discuss the inter-related concepts within the middle range theory. What aspectsof the nursing meta-paradigm are addressed

in the mid-range theory? How will this theory guide your assessment/perception of the POI? How does this mid range theory relate to your grand theory?

Complexity science: how does complexity science relate to your POI? Depending on the nature of your POI, this conversation may have different foci for different

students. For example, if your POI is glycemic management of the peri-operative patient, this discussion would center on the complex responses of the human body as a

Complex Adaptive System. If your POI is focused on a policy change issue, the focus may be on organizational complexities with communication, change, etc.

Ethical framework: you should discuss your specific POI in the context of ethical principles. Basic principles include autonomy, justice, beneficence, and non-

maleficence. Is your POI in violation of a core ethical principle? Is it possible it may be in violation? What ethical principles do you need to be sure to safeguard?

Conclusion: this section should not contain any new information but should only provide a summary of what what discussed in the paper.

Since you are going to be discussing theories within this paper, it is very important to provide the PRIMARY source when referencing the theory either in text and on

the reference list. Who wrote the theory? And when was it published? Answer those questions and that is who the citation should be. For example, Roger’s theory of

unitary beings should be cited to Rogers, not to a text book or journal article that discusses the theory. If you are having problems locating the original source for

the theory, refer to the reference lists with articles or in books that your are reviewing. You will almost certainly find the original source referenced within the

article or textbook.
This is a scholarly assignment which means it should be liberally referenced with peer reviewed literature. Support what you say with evidence from the literature.

There is no “hard and fast” rule for the number of references to include, but for an assignment of this magnitude, I would say that 10-12 references would be a

minimum. There a number of potential references provided for you in the Required readings and resources of this Lesson’s Unit. This should be a great start, but

searching the literature for further resources is also going to be required.
