Alvita is a frail 89-year-old woman residing in a nursing home. She is able to move slowly around the residence with the use of a walker, but appreciates when her daughter is there to hold her arm and walk alongside her. When one of the health care staff changes Alvita, her daughter helps. Alvita’s incontinence has progressed, particularly over the last six years since she has resided in the nursing home. Alvita can smile at her lack of bladder control, however, and says that her incontinence really began when she was a young woman, just after the birth of her second daughter.

  1. Alvita’s mobility is limited. How does this affect continence in the elderly?
  2. Shortly after the birth of her second daughter, Alvita experienced mild incontinence, particularly after laughing or coughing. What was she experiencing? What is the pathophysiology behind this type of incontinence?

Phenomenology: The goal of the phenomenological approach to qualitative research is to describe accurately lived experiences of people, and to not generate theories or models of the phenomenon being studied. Since the primary source of the data is the live world of the individual being studied, interviews are the most common source of data collection.

Grounded theory: The purpose of the grounded theory approach is to discover social-psychological processes. Feature of the grounded theory include, theoretical sampling and constant comparative method. Theoretical sampling refers to sampling decisions made throughout the entire research process in which participants are chosen based on the knowledge of the topic and based on emerging study findings.

Ethnographic research: The goal ethnography is to learn about culture from the people who actually live that culture. A culture can be defined as a society, a community, an organization, a spatial location or a social world. The process of ethnography is characterized by intensive, ongoing, face to face involvement with the participants in the study.

Phenomenology vs. Grounded theory: The two are similar and useful for nursing research in that they produce results that are useful in precipitating change and creating substantial realistic and accurate data. The differences are in the types of research questions that are asked, the methods used and to some extent the final products. Based on real life experiences vs. theoretical experiences with subjects that have knowledge of the topic at hand yet may not have experienced the topic itself.

Nurse leaders should be integrated as managers. “Nurse Managers are an essential contributor to excellence in nursing practice, staff nurse engagement, and patient satisfaction, and are responsible for creating and supporting an environment to develop staff nurses into leaders (Keys, 2014).” Some leaders struggle with being a leader and being a manager. I once had a manager that all she wanted to do was manage and not be a leader. She had a tone, and people were afraid to go to her to talk about any issues they had because they were afraid of getting in trouble. I had another nurse, who is a charge nurse, who all he wanted to do was move up the latter and continue being everyone’s friend. He would not have those conversations that some nurses need to have. He would not manage his team and that caused issues with upper management because then they had to pick up his slack.

There has to be a balance between the leader and manager style. Effective leaders should be able to have great communication with all staff, be knowledgeable, and approachable. They can manage a unit, and still be friendly but there needs to be a line between leader and friend.You cannot be one without the other, but too much of one can cause more harm than good. Leadership characteristics should be integrated into every step on the management process to help nurses with the transition. (Marquis & Huston, 2017)

One way we can learn how to appreciate what role nursing organizations have is by exploring the many benefits they have to offer when you acquire membership. For example, some join to learn what’s going on and stay current in their field or specialty. Others want to network or enjoy the peer recognition that comes from being an active member of a major organization. You can attend educational conventions and meet nurses of the same or even different specialties. You can pick their brain and listen to different ways of how others are handling some of the same issues you face. Most importantly, nursing organizations also play a role in the legislative process; just not as much as we would like it to. Which is why it is essential that nurses become involved. The healthcare system is in need of change, it is important now more than ever that nurses step out of their comfort setting and lobby and advocate in the realm of policy, law and politics. The problem is that nurses most likely don’t feel prepared in this area. Successful policy advocacy depends on having the power, the will, the time, and the energy, along with the political skills needed to ‘play the game’ in the legislative arena (Abood, 2007). Nurses are the frontline providers of care, who better than to develop these policies and laws that are directly impacting patients and their care.

This week, we will discuss the research design and sample for your nursing clinical issue. The research design flows from the research question and outlines the plan for the study that will answer the research question. The design identifies the major components of the study. It is important to remember that there is no one best design for a research study.

After you review the designs, describe which research design you would expect to find when searching for evidence relevant to your own research question from Week 2. Why? Explain your answer.
The most common sampling method is the convenience sample; therefore, many of the studies that you find for evidence use this sampling method. What are the implications for using a convenience sample on the way you interpret and use the findings?

The research question drives the research design. There is no one best research design, it is important to determine the design based on your plan for the study. Compare qualitative and quantitative research designs and outline when one would be more appropriate than the other. Let me give you an example from my practice, I facilitated large scale, oncology trials, measuring one cancer treatment against another, these needed a quantitative design because we collected large quantities of data to analyze and determine which treatment was more effective. On occasion, we also wanted to see what the patient’s experience was during their treatment, these studies involved much smaller sample sizes and we wanted to know the quality of the experience, so we collected qualitative data, in the form of interviews.

Now, reflect on your chosen nursing clinical issue. Summarize why you think one research design will work best for your research question.

Considering the heightened importance of evidence-based practice in healthcare settings, incorporating evidence-based practice into the nursing curriculum, especially in baccalaureate programs is essential because this is a first step to prepare students for their professional role as an RN, and the undergraduate nursing students are the ones who will spend the most time with patients at their bedside providing direct care” (Martin, 2011). Another essential component of EBP integration into practice is that new graduates are more passionate and interested in changing or improving current practice skills and techniques and have fresh eyes to see what is not working or what could be improved upon. Older nurses who have been working for a while may have seen changes already and do not want to develop new ideas. Perhaps they felt in the past that the changes did not affect patient outcome and so decided to go back to the old ways. Perhaps the older nurse is too job tried to accept new changes and are not comfortable with the new way of doing things. As a new graduate, it is our responsibility and role to implement these changes if the evidence points to patient benefits and lower cost reduction to the health care facility. There are many ways of facilitating changes but testing out the culture and climate of the unit is very important and making sure the majority of the staff will buy into the changes. You can present all the evidence in the world but if you staff is not willing or nervous about the changes, the EBP project will not go far.
