I have picked to discuss option #2 which is the patient with Crohn’s disease.

1. I would respond to the client by further assessing the situation. I would do a spiritual assessment, more specifically the RCOPE assessment (Weber, 2018, pg. 210) This assessment would help me assess how the pt is coping with their situation. I would also keep in mind that sometimes spirituality has a negative effect and can cause depression but it can also be positive and can improve the patient’s quality of life and reduce stress. (Baldacchino, et al., 2013, pg. 229)

2. It stated above that the patient was distressed and wasn’t able to come with the terms of the condition. Some identified risk factors would be the risk for depression and decreased quality of life. There also might be a risk for decreased appetite due to stress, and the pain this disease can cause with the digestive track. Some interventions that could be done is to offer for spiritual care or a counselor to come talk to pt. I also might ask if they wanted us to contact a specific spiritual leader that they were familiar with so they cold talk about their spiritual distress. I might also try to help them understand the disease process if they don’t understand it and might ask if it would be possible to get a dietary consult or a dietitian who could go over the diets or foods that would settle well with the patient with crohn’s disease.

Experimental research design is a type of quantitative research design that is highly controlled and study cause and effect with independently and dependent variables (GCU, 2018). The primary goal for experimental research is to provide strong evidence for cause and effect relationships.

In contrast to experimental research, nonexperimental research involves variables that are not manipulated by the researcher and instead are studied as they exist. The use of nonexperimental research in some cases, would be unethical to randomly assign individuals to different treatment conditions (Johnson, R.B. & Christensen, 2008). Example of this is that one could not study the effects of smoking by randomly assigning individuals to either a smoking or nonsmoking group for a given number of years. The only ethical way is to identify a group of smokers and nonsmokers and compare the differences in their current state of health, how long people had smoked, their gender, age and general health level. To do this would be important because the researcher cannot take for granted that the group are comparable in aspect other than smoking behavior. This contrasts with experimental research groups which due to the process of random assignment, start out equal in all respects except for the treatment condition in which they are placed.

Unfortunately, Mrs. Fox’s right leg could not be saved, and a surgical amputation was performed below the level of the knee.  At that time, the patient also displayed impairments to the following: swallowing solid foods, speaking words and full sentences, anterograde memory, fine and gross motor function of the right upper and lower extremity.

Mrs. Fox spent 2-3 weeks in inpatient care receiving all necessary rehabilitation services with good improvement in objective measurements.  However, it was determined that the patient was not yet prepared to return home safely with her family.  The necessary paperwork was processed so that the patient could be transferred to Helen Hayes Hospital, a skilled nursing facility, where she could continue to receive rehabilitative and nursing services.

Prior to her accident, Mrs. Fox had a 15 year history of cigarette smoking.  Her physician strongly advised her to cease smoking due to its negative effect upon tissue healing and overall negative effects upon her health.  During a rehabilitation session, the patient reported that her situation had left her feeling depressed, particularly because of her difficulty communicating with her husband and 2 children.  She also expressed fear that she would never walk “normally” again or be able to do things such as brush her hair or feed herself like she did before.  She stated that the only time she felt “happy” lately was when she was painting in the common room.
