The 2010 Patient Protection and Affordable Care Act (ACA) is a landmark legislation that is considered by many to be the most significant advance in the United States healthcare policy since the passage of Medicare and Medicaid in 1965. However, the ACA is not without controversy stemming from opponents all along the political spectrum. Some believe the law has gone too far, others not far enough.

Aside of ACA, an alternative approach to healthcare delivery has been referred to as the “single-payer” system. Research the single-payer approach to healthcare delivery in the United States using your textbook, the Argosy University online library resources, and the Internet.

Address the following:

  • What are the strengths and challenges presented in this solution?
  • Identify any omissions in this solution and discuss how they should be addressed.
  • Identify any changes that you would suggest.
  • Based on your current situation in terms of healthcare coverage (insured by an employer, covered under a parent’s or spouse’s policy, receiving government benefits, uninsured, etc.), indicate how a single-payer plan would affect you personally. A case base reasoning system is a type of “expert system.” It attempts to match the facts on hand to a data base of prior “cases.” When a case-based reasoning system finds one or more in its data base that closely matched the facts at hand, it then evaluates and reports the most common outcomes. Given enough cases, such a system can prove very useful. Even better, if a case-based system automatically captures cases as they occur, then it will become a powerful tool that continually fine-tunes it results as it gains experience.  Amazon.com relies on just such a system to refer books to its customers. Like many  e-commerce sites, Amazon allows visitors to search for, buy, and review books. Amazon.com takes its database interactivity a step further. Given a particular book title, its case-based reasoning engine examines all past sales of that book to see if the customers who bought that book to see if the customers who bought that book share other book purchases in common. It then produces a short list and presents that list to the user. The overall effect approaches that of a sales clerk who says “Oh! If you like this book, then you’ll really like reading these as well.” However, Amazon’s system has the experience of hundreds of millions more transactions than the most wizened and well-read sales clerk. Equipped with this information, a customer may consider purchasing additional books; better information increase the customer’s confidence in the purchase and encourages sales.

  • Inventory the organization’s data as the first step in policy implementation.
  • Read the Federal Register information on HIPAA.
  • Focus on HIPAA as a business process issue.

Mark Totten is the chief compliance officer at Peachtree. Compliance with HIPAA Privacy is crucial to maintain the accreditation of the hospital. Three basic areas that HIT professionals must be concerned with include:

  • the release of information (ROI). JCAHO requires that healthcare organizations develop role-based access to information. Organizations must review the types of people who access information and limit their access to the information needed to perform their job. Misuse of blanket authorizations can result in non-compliance with the HIPAA Privacy Rule;
  • protection of privacy, maintenance of confidentiality, and protection of data security. This includes the physical and electronic protection of the integrity, availability, and confidentiality of computer-based information and the resources used to store, enter, process, and communicate it; and 
  • management of sensitive health information. Strict compliance of state and federal laws govern the release of health information.
