Purpose

You are to create a Design for Change proposal inclusive of your Practice Issue and Evidence Summary worksheet from your Capstone Project Milestone 1. Your plan is to convince your management team of a nursing problem you have uncovered and you feel is significant enough to change the way something is currently practiced. In the event you are not currently working as a nurse, please use a hypothetical clinical situation you experienced in nursing school, or nursing education issue you identified in your nursing program.

Course Outcomes

This assignment enables the student to meet the following course outcomes.

  • CO1: Applies the theories and principles of nursing and related disciplines to individuals, families, aggregates, and communities from entry to the healthcare system through long-term planning. (PO #1)
  • CO2: Proposes leadership and collaboration strategies for use with consumers and other healthcare providers in managing care and/or delegating responsibilities for health promotion, illness prevention, health restoration and maintenance, and rehabilitative activities. (PO #2)

  • Create a proposal for your Design for Change Capstone Project. Open the NR451_Milestone2_Design_Proposal_ template in Course Resources to create your paper. You will include the information from Milestone 1, your practice issue and evidence summary worksheets, as you compose this proposal. Your plan is to convince your management team of a practice problem you have uncovered that is significant enough to change current practice.
  • The format for this proposal will be a paper following the Publication manual of APA 6th edition.
  • The paper is to be four- to six-pages excluding the Title page and Reference page.
  • As you organize your information and evidence, include the following topics.
    • Introduction:Write an introduction but do not use “Introduction” as a heading in accordance with the rules put forth in the Publication manual of the American Psychological Association (2010, p. 63). Introduce the reader to the plan with evidence-based problem identification and solution.
    • Change Model Overview: Overview of the ACE Star model (the model we have been discussing this session); define the scope of the EBP; identify the stakeholders, and determine the responsibility of the team members.
    • Evidence: Conduct internal and external searches of evidence; integrate and summarize the evidence summary worksheet from Milestone 1; develop a recommendation for change.
    • Translation: develop a hypothetical action plan; include measurable outcomes, reporting to stakeholders; identify next steps and disseminate the findings.
    • Conclusion: Provide a clear and concise summary, inclusive of the problem issue, the five points of the ACE Star change model; and ways to maintain the change plan.
  • Citations and Referencesmust be included to support the information within each topic area. Refer to the APA manual, Chapter 7, for examples of proper reference format. In-text citations are to be noted for all information contained in your paper that is not your original idea or thought. Ask yourself, “How do I know this?” and then cite the source. Scholarly sources are expected, which means using peer-reviewed journals and credible websites.
  • Tables and Figures may be added as appropriate to the project. They should be embedded within the body of the paper (see your APA manual for how to format and cite). Creating tables and figures offers visual aids to the reader and enhances understanding of your literature review and design for change.

As an advanced practice nurse, we will sooner or later come across an issue or more than one issue that will cause us moral distress.According to Westrick, moral distress occurs when there is a mismatch or disequilibrium between moral obligations and principles, and the ability to implement them, (2014 p. 310).As a nurse practitioner working in a hospital setting we will come across issues of how to advocate for our patients when it comes to quality of life rather than quantity of life.“Every day nurses make great moral decisions in their workplace, but in practice they cannot always act according to their moral obligations,” (Williams, Wang, & Kitchen, 2014,p.7).One thing that we must assist the patient and family member is to understand and choose the proper treatment.The physicians should speak to the patient and family member about the disease and the prognosis, also be able to answer questions about death, and pain relief.

Ashford General Hospital is a 263-bed regional hospital located in California, serving its community for more than 50 years. The hospital maintains the only 24-hour emergency department in the area and an “extended hours” urgent care clinic. Similar to other hospitals in the United States, Ashford General Hospital is encountering a nursing shortage. Sixty-eight percent of the nursing staff is over the age of 45, facing retirement. The retention rate on nurses is 61%, compared to 65% nationwide.  Many of the nursing staff find the work too physically demanding and have a feeling of emotional burn-out as well.  In the past two years, the hospital has used both per diem nurses and traveling nurses who sign short-term contracts to fill individual shifts and accommodate short-term staffing needs arising from staff vacations or medical leaves.  This has not only driven up personnel costs but also resulted in lower scores on patient satisfaction surveys. Ashford General Hospital faces significant challenges in nurse staffing ahead as it grapples with these issues, and the hospital board is very concerned. They know there must be some changes made in order to prevent a major financial and human resources crisis in the future. As the newly hired CEO, you are asked to present a proposal in the next board meeting. 

Instructions for the final paper:

For your Final Paper, you will create a 10 to 15 page proposal (excluding title and reference pages) for the Ashford General Hospital Board of Directors. In your proposal, you will:

  • Include an executive summary.
  • Research and describe solutions implemented at five other hospitals in the US that were dealing with these same issues.
  • Based on your research, describe two solutions that are the most viable for Ashford General Hospital to implement within the next two years.
  • Create a total of two stakeholder group analyses for two solutions you select. Each analysis should include at least five stakeholder groups involved (e.g. patients, hospital executive administration, accounting, Human Resources (HR), marketing, third-party payers, etc.). As part of your analysis, address the following questions:
    • Who is impacted?
    • What change processes may be required?
    • What fiscal impact would occur?
    • What are the ethical, legal, and diversity risk factors involved?

– Describe the issue, define terms, characteristics and context of the issue in Canadian
nursing practice. In this paragraph you do not provide supporting arguments
here, but rather prepare to put forward a thesis statement 

– End the paragraph with an articulated thesis statement sentence. 

Your aim is to write the thesis statement (your opinion) so that it is clearly worded,
concise, and identifies the dilemma of the topic or the aspect of the topic
that is controversial. The thesis statement is the focal point of your essay. A
thesis is a proposition put forward and supported by arguments. A well -developed
thesis statement takes an assertive stance on the identified issue and is debatable
(i.e., there is an opposite thesis to the one that you have proposed). The
thesis statement is to be the final sentence of this second paragraph. 

D. Analyzing the Issue
(Paragraphs 3, 4 and 5)  

Each of the next three paragraphs is a separate reasoned analysis and evidence that
supports the thesis. Paragraphs 3 and 4 provide different arguments to support
