A dependent variable is the outcome variable. It takes on different values in response to the independent variable (Schmidt, 2018).

An extraneous variable is a variable that can influence the relationship between the independent and dependent variable; can be controlled either through research design or statistical procedure; were not foreseen or known at the beginning of the study. Another definition of an extraneous variable is that it is any variable that you are not intentionally studying in your experiment or test. When you are conducting an experiment, you are looking to see if one variable (the independent variable) has an effect on another variable (the dependent variable ). So then when you conduct the experiment, at the end you will get results, however sometimes variables that you didn’t expect to be the outcome of the experiment may be undesirable and these are known as extraneous variables (Statistics How to, n.d.).

One way researchers try to control extraneous variables is with participant variables. Participant variables minimizes difference between participants. For example, gathering a group of people with the same IQ or their stage of development such as age (Tutor 2 U, n.d.).

A second way researchers can control extraneous variables include situation variables. Situational variables congtroll the setting where the experiment takes place, such as keeping light, sound, and temperatures levels consistent (Tutor 2 U, n.d.).

Discussion 1

For this weeks discussion I will be examining the Self-Report method of data collection and its shortcomings. When using a Self-Report method, the study participants relay their subjective feelings and experiences through interviews or surveys and questionnaires (LoBiondo-Wood & Haber, 2014, p.279). This method of data collection relies on participants to be honest and forthcoming, and there in lies one of the shortcomings.

In their evaluation of data collection methods for drug use Linas et al. (2016) note that while self report is considered a valid and reliable method of measurement for drug use it also faces the problems of recall and social desirability biases (p.1). LoBiondo-Wood and Haber (2014) further elaborate that people will give answers that are desirable to the researcher or that paints them in a favorable light (p.279). LoBiondo-Wood and Haber (2014) further note that the respondent burden poses a risk to research validity. Respondent burden occurs when the excessive length of the interview or survey that leads the respondent to “give up” and provide inaccurate responses to “escape” leading to skewed or incomplete data (p.279).

Self reporting is a common and useful tool in the clinical environment as it allows us to assess subjective measures of the patients experience such as pain, fear, and satisfaction with care. Self reporting is also frequently used in research to answer these same subjective questions. For example, if a researcher wanted to study the quality of life of adult amputees in the first 2 years following amputation they could utilize the self report method of data collection at one, three, or six-month intervals and possibly use this data to help future amputees by providing more assistance, education, or support during difficult time periods or stages following the loss of a limb.

Data collection is a big part of research. Researchers utilize information gathered to study phenomena or make changes and find solutions. Depending on the type and goal of the research, different data collection methods are used to get results. There are two types of data collection methods that is primary and secondary. Secondary data is usually already in circulation and can be found in books, journals or newspapers written by other people while primary data is usually data stemming from the person who carried out the research study (LoBiondo-Wood & Haber,2014). The purpose of the discussion board is to analyze one data collection method and discuss its pitfalls.

In my health care facility, questionnaires are used as a form of data collection to gauge patient’s experiences during their stay at the hospital. The questionnaire asks patients to rate their experience from excellent, moderate to poor. At the bottom of the form, patients are asked to include strengths, weaknesses and areas of improvement. Patients are not required to fill their names on the questionnaire and therefore are more likely to be open and honest in their responses. The questionnaires are given to the patients during admission and therefore are given plenty of time to answer the questions before discharge. There are various topics covered on the questionnaire including nutrition, environmental services, doctors and nurses’ availability, atmosphere on the floor. On the downside, questionnaires are impersonal and thereby a patient’s feelings and emotions are unknown. With questionnaires, there is a risk of getting untruthful answers (McCormack, Friedrich, Fahrenwald & Specker, 2014). Some patients may not be honest in their responses. Questionnaires may be interpreted differently by different individuals. For some patients, one question may have a different meaning than the other patient thereby resulting in inaccurate results. With questionnaires, there is no guarantee for compliance from the respondents and that may affect the results of the study (McCormack, Friedrich, Fahrenwald, & Specker, 2014).

Different methods of data collection have both advantages and disadvantages. As a researcher, it is important to check the credibility of the sources and gather as much information as possible to get accurate results. The process of data collection can be time consuming but it’s the end result that matters. Research studies are the basis of innovations in medicine and healthcare in general.

It is pertinent that medical personnel have access to a patient’s lab results, imaging, assessment flowsheets and medication records, in order to appropriately care for their physical needs. Health information technology allows for nurses and other medical staff, to obtain the information needed to accurately treat and sometimes cure our patient’s illnesses. The EHR, or electronic health record, was created and mandated throughout health care facilities across America, to provide healthcare professionals the tools needed to help heal their patients. (Wallace et al, 2013) But as with any new technology, there are complications that arise that can have negative consequences.

There are two types of IT (Information Technology) related incidents that can have severe negative outcomes. The first of these IT issues is to have human-computer problems. When a human-computer incident occurs, it means that there was an interface issue between the human user and the health information system that caused the human user to use the system incorrectly. (Wallace et al, 2013) The second type of IT incident is a computer related issue, such as a glitch, that causes problems in how the equipment and software function. (Wallace et al, 2013)

Wallace et al (2013), gives an example in a case study of how a hospital switched from using paper MARs, to eMARs (electronic medication administration records) and a computer related incident occurred causing a patient to be overdosed on morphine and to go into respiratory arrest. When the situation was debriefed, the cause of the overdosing was found to be that the eMAR was not displaying the entire medication order; therefore the nurses were not able to see the administration instructions in their entirety, causing the nurse to administer to high of a dose. The patient had to be intubated and resuscitated during the overdose, but was able to be saved. Because this type of IT incident was computer related, the IT developer from that hospital was able to easily fix the problem within the eMAR. The potential consequences that this patient faced due to this computer related incident and overdosing could have been as severe as death. The patient could have developed brain death from the hypoxia, or could have had an ischemic stroke or MI. (Rull, 2017)

Nursing leaders today, can establish a culture of safety related to health IT by, reinforcing the practice of the five rights of medication administration that every nurse learns in nursing school, and by enforcing that order clarifications be performed before medication is administered at all times. (Wallace, 2013) If the nurse in the case study was not sure of which morphine dose to give, or would have even questioned the orders, it might have prevented the patient from suffering these harsh consequences. When a nurse practices her five rights of medication administration, he or she has to pay close attention to the medication order itself as well as the medication packaging in hand. He or she would have in this case had to get an order clarify performed due to the parameters not being clearly seen on the medication orders. The nurse manager is responsible for making sure that the nurses practice using these five rights in order to prevent medication errors. He or she may sometimes have to enact written discipline on those nurses who wish to not obey nursing protocol.

Health information technology has helped bring the world of medicine a long way in recent years. It is not without its flaws, but if medical staff is trained to use it appropriately and wisely, it can help make the health care experience for our patients much better.

In the rapidly changing world of technological advances, and the application of these new technologies into the world of healthcare, questions regarding the safety of the application of these technologies become an issue. The use of electronic medical records, and their ability to access information quickly and communicate to care team members almost simultaneously (Burkhardt & Nathaniel, 2014), have been in use for my entire ten-year career in healthcare. Legal and ethical obligations to the protection of the safety of these records requires safeguards to be implemented (Burkhardt & Nathaniel, 2014). What if the safeguards fail or are not correctly implemented? The purpose of this post is to examine and identify the potential issues related to health information technology and discuss nursing leader’s role in establishing a culture of safety related to health information technology.

The two main issues in health information technology arise from either computer-related issues or human-computer issues (Wallace, Zimmer, Possanza, Gianinni, & Solomon, 2013). In computer-related issues, an issue with (1) system interface, (2) system/software configuration, or (3) software function cause a disruption in the transmission of data and inhibits system/software functionalization (Wallace, Zimmer, Possanza, Gianinni, & Solomon, 2013). In human-computer issues, human error, in areas such as data entry, patient identification, medication selection errors, or failure to operate or understand the system properly, creates the issues in the health information technology (Wallace, Zimmer, Possanza, Gianinni, & Solomon, 2013).

Wallace, Zimmer, Possanza, Gianinni, & Solomon (2013) present a case study in which a computer-related issue with the electronic medical record, as well as a possible human error, result in an adverse event of a respiratory arrest for a patient that required intubation and resuscitation. The patient was ordered extended-release morphine every twelve hours and immediate-release morphine as needed, but the electronic medication administration record displayed both orders as morphine with the dosing information cut off and not visible (Wallace, Zimmer, Possanza, Gianinni, & Solomon, 2013). The patient received both doses at the same time, resulting in the respiratory arrest and the resultant follow up included the health information technology developer to include the drug formulation for morphine as well as to allow the electronic medication administration record to display and prevent the cut off of dosing information (Wallace, Zimmer, Possanza, Gianinni, & Solomon, 2013). The computer-related error is obvious by the inability for the electronic medication administration record to display all pertinent information related to the morphine orders, but I feel an error in judgement is displayed by the nurse in giving both the extended-release and immediate-release morphine at the same time. The case study presented by Wallace, Zimmer, Possanza, Gianinni, & Solomon (2013) does not fully allow for complete mental visualization of the electronic medication administration record, but does indicate the nurse mistakenly gave both drug formulations. I personally would not feel comfortable with giving both the extended-release and immediate-release morphine at the same time, nor would I be comfortable with the presentation of the orders on the electronic medication administration record and would look to seek clarity from the ordering physician. The resultant consequences for the patient of the respiratory arrest with intubation and resuscitation (Wallace, Zimmer, Possanza, Gianinni, & Solomon, 2013) are just one of the potential consequences, as death, subsequent cardiac arrest, coma, hypoxemia, or at the very least increased somnolence could have occurred.

Finally, nurse leaders can establish a culture of safety towards health information technology by guiding, instituting, and practicing strategies that provide monitoring, measurement, and improvement to concerns of patient safety in regards to health information technology (Singh & Sittig, 2016). Encouraging the reporting of issues with health information technology (Wallace, Zimmer, Possanza, Gianinni, & Solomon, 2013) and by complying with institutional precautions and safeguards, nurses can foster a culture of safety in health information technology.

Before reading the article written by Louis Menand, I wasn’t sure what the real purpose of why I chose to pursue a college degree was. Out of all three theories presented, Theory 3 seemed to depict the real reasoning behind why I chose to attend college spot on. On page 5, paragraph 6; Menand states “College is basically a supplier of vocational preparation”. For someone who isn’t a liberal arts student like myself, I have only one reason to attend college; to become a nurse. In a job field that requires a fine tuned skill set, and specialized training, a class such as English 101 isn’t nearly as important as a class on the anatomy of the body is to a nursing student. On a course to a career that is as rigorous and competitive as nursing, you must be job focused and follow a clear set in stone path. Unlike in Theory 2, I don’t get satisfaction and motivation from what I learn, at the end of the day I will only feel satisfied when I know I’m prepared for my job outside of college. As much as I enjoy good grades, they’re not my motivation either, the courses I need to complete to get a nursing degree are very important, as well as what is taught in them. If I were to take any class just to receive a good grade my path to becoming a nurse wouldn’t be attainable, like Theory 1 students. The information I obtain from the classes I take must be applicable in my line of work. Boise State University upholds my purpose for coming to college because the university allows students to “pursue excellence with diligence, perseverance, and continued improvement.” found in the statement of shared values. For a student who identifies with Theory 3, we must have perseverance and diligence so we will not waver from the specialized path we are set on. But, to pursue a career in a specialized job such as nursing, you must be able to continually improve and add on to the skill set you are creating to get to the job you set out to achieve. As I continue on my pursuit of a degree in nursing, those three characteristics will be critical to my excellence in the future of my career.

Of Menand`s three theories, the theory that most closely matches my own reasons for seeking a university education is theory two. Although, after reflecting on my own reasons, I am a mixture of all three theories. This perspective, may be hard to grasp as each theory contradicts each other. I am at college to not only get a degree and good grades in the process, but also to learn and obtain skills that I will be able to apply to a job. Although I carry many traits from all three theories, theory three is the one I hold the least characteristics of. I know what my general direction of career path is, but I do not know the exact path. I would love to be in some part of marketing and use Spanish to reach people I normally would not be able to if I do not obtain a Spanish degree (which would ultimately help my fluency). I am in college to learn more about the world around me, gain valuable knowledge, and to better my future. Grades use to be everything for me, but after being diagnosed with several very serious concussions, school became very hard for me. I use to be able to comprehend material and take a test on it accurately. For several years in high school I struggled with trying to reteach my brain how to learn and pull that knowledge for exams. Growing up I was at the top of my class. As theory one states “College is, a four-year intelligence test” (Menand), but in my own experience, I know that it is not an intelligence test. I give all my time and effort into my studies. I am smart. I know I am smart, but my scores sometimes do not show that. My grades may reflect the many concussions, but not my intelligence. I have come to the university to better myself and find my niche in the world. I may not have all the answers of what I want to be when I grow up, but I do know that getting a higher education is necessary for me to succeed in life.
