Ashford General Hospital is a 263-bed regional hospital located in California, serving its community for more than 50 years. The hospital maintains the only 24-hour emergency department in the area and an “extended hours” urgent care clinic. Similar to other hospitals in the United States, Ashford General Hospital is encountering a nursing shortage. Sixty-eight percent of the nursing staff is over the age of 45, facing retirement. The retention rate on nurses is 61%, compared to 65% nationwide. Many of the nursing staff find the work too physically demanding and have a feeling of emotional burn-out as well. In the past two years, the hospital has used both per diem nurses and traveling nurses who sign short-term contracts to fill individual shifts and accommodate short-term staffing needs arising from staff vacations or medical leaves. This has not only driven up personnel costs but also resulted in lower scores on patient satisfaction surveys. Ashford General Hospital faces significant challenges in nurse staffing ahead as it grapples with these issues, and the hospital board is very concerned. They know there must be some changes made in order to prevent a major financial and human resources crisis in the future. As the newly hired CEO, you are asked to present a proposal in the next board meeting.

Victim services

The victim I selected the abuse of a woman in her 80s that resided in a nursing home (Laureate Education, 2012). Her son had reported to the police that he noticed that there were bruises on her arms and her torso (Laureate Education, 2012). He also noticed changes in her behavior and then he decided to remove her from that nursing home (Laureate Education, 2012).

One program that is offered here in Virginia, for this type of victim is Adult Protective Services (APS) (Adult Protective Services (APS), 2016). If these protective services are needed and are accepted by the adult, a local APS worker may arrange for health, housing, social, and legal services for the adult (APS, 2016). This is supposed to stop the mistreatment and to prevent further mistreatment of the victim (APS, 20156). Other services that are offered through APS are home-based health care, transportation, nutrition services, and legal services (APS, 2016). Another program is the Virginia Coalition for the Prevention of Elder Abuse, Inc. (VCPEA). The VCPEA helps encourage the development of services and programs to protect the elderly with disabilities who are abused or neglected (Virginia Coalition for the Prevention of Elder Abuse, Inc. (VCPEA), 2016). They also promote the improvement of quality of services to vulnerable adults and they also promote a greater public understanding and awareness of elder abuse (VCPEA, 2016).

I think these programs are adequate. They help the elderly try and cope with what has happened. They help the elderly get back on their feet after this type of incident happens. Also, providing legal services so that the elderly won’t have to worry about anything, so they can try and go on with their lives.

A service that I think should be available would be something to help the victims get compensated for what they have been through. When a person is in a nursing home they are paying for the services there. They didn’t ask to be abused and they should get reimbursed for everything that they had paid in during their entire stay there and maybe more. There needs to be a program available that can help out with this.

The Case Assignment

Bobby Langdon is the current president of Acme General. It is a 300-bed hospital serving a community of 70,000 people. He has just completed a meeting with the hospital administrator, Susy Miller. The meeting was called to address to find an acceptable solution to an apparant conflict between Ms. Miller and the chief of the Bariatric Program, Dr. Kincaid. The problem: The Bariatic Program manager is scheduling the various surgeries in such a way that the providers do not feel they can adequately perform these complicated procedures. They also feel there isn’t proper time allowed between procedures to set up the surgical suite for the next case. The providers feel that the scheduling issues are resulting in poor patient outcomes. It has also been pointed out that the program manager shows favoritism toward some of the providers and allows them more time in the OR and between cases. The problem escalated when Dr. Kincaid fired the program manager after numerous complaints from providers, staff, and patients. The manager talked to Ms. Miller who over-ruled the doctor. She cited that non-clinical staff could not be fired by the program director. Some years back majority of elderly people died in their home, but with the new trends of medical technology today, the end of life care has changed. Presently by research, most Americans are in hospitals or nursing homes for rehabilitation or for palliative care (End-of-Life) because of the inability to take care of themselves due to illness. Also, they may unexpectedly admit to the hospital with a life-threatening illness. Most elderly people do not die in their own homes because they lack caregivers, their family may be too occupied with work/school activities. So, they opted  to help their loved ones out to go to a nursing home or assisted living for either rehabilitation or palliative/hospice care. According to BMC Medicine, there is some evidence showing better results on psychological, social, and holistic measures of well-being in the last few weeks or days of life for patients dying at home compared to the hospital.  Some research claimed that there may be no way to control pain in a patient dying at home which may be the reason their loved ones take them to the hospital or nursing home for proper pain management. Also in one of the research done in the UK, it was reported that there are lots of grief among relatives of patients who died at home. This may be why we have seen a decrease of elderly deaths at home. Most elderly people prefer to die at home because of the familiarity with the environment where they maintain control and greater freedom than in a hospital or even inpatient hospice. It is very difficult to deal with a patient that is in end-of-life care because you are not only caring for the patient but also the family. According to the Nursezone research from the California Healthcare Foundation highlights how far apart people’s desires are from reality. Seventy percent of respondents said they would prefer to die at home, but in 2009, only 32 percent of deaths in California occurred at home. Nearly 80 percent of those investigated said they would prefer to talk with a doctor about end-of-life care, but only 7 percent really had that conversation. With these results it can be hard to help a patient decide about care if they are not sure about what they want. It is very important to educate my patient and talk with them on the options chosen by the patient or the family. For instance, if the patient is like about 85 years and above, and the family are demanding for CPR or still want the patient’s status remains a full code, I would have to sit them down, explain, and make them aware that even with all the measures to save his or her life, there will be no tangible quality of life and performing CPR on elderly patient would be very traumatic to the patient. As a nurse caring for a palliative patient, advocacy is significant.
