The AACN essential I have chosen to elaborate on is Essential II. Essential II: Basic Organizational and system Leadership for Quality Care and Patient Safety. I consider this essential extremely important in my current and future nursing practice. “As a member of a healthcare team, baccalaureate graduates will understand and use quality improvement concepts, processes, and outcome measures. In addition, graduates will be able to assist or initiate basic quality and safety investigations; assist in the development of improvement action plans; and assist in monitoring the results of these action plans within the clinical microsystem, which is embedded within a larger system of care” (AACN, 2014 p 13). As a clinical nurse, it is of the utmost importance for me to maintain patient safety. When patient safety is not achieved, then it is imperative to improve and investigate ways in which patient safety can be enhanced.

Essential II is currently being used in my professional nursing career during our UBT (Union based team) meetings. During these meetings, we discuss ways in which we can improve different things in our unit; one of them being patient safety. By having a better understanding on quality care and patient safety, I believe I will be better qualified to improve safety to the patients in my unit.

I believe all the nine nursing essentials will have an impact on my nursing career because they are all crucial to our nursing practice. Nursing as we know it, would stop evolving without these nine essentials. “The demands of twenty-first-century health care services will continue to test the nursing profession’s ability to maintain high caliber care that meets contemporary societal needs. Historically, the nursing profession has consistently demonstrated its ability to adapt to changing and varied health care needs. In viewing and reading the media piece on ” Nursing Timeline of Historical Events”, I have come to realize that many of the commodities we have now in nursing and the healthcare industry as a whole, are due to the hard work, research and extensive lobbying of our nursing pioneers. Understanding how individuals such as Harriet Patience Dame, Florence Nightingale and Mary Mahoney had to adapt to circumstances that involved lack of equipment, consequences of war and discrimination in order to change nursing.

Three trends in Nursing that have influenced my perspective of nursing practice are Access to Nursing Education, Technology, Nurse Regulation. Access to Nursing Education has evolved drastically. Some of our pioneers didn’t even have official education. They used their primitive tools and resources to nurse individuals back to health. Presently, there are numerous institutions which offer Associate’s and Bachelor’s degree education. These degrees can be obtained from State Universities, Private colleges, Community Colleges and the list may go on. As patient needs have risen, so has the advancement in technology. Currently there are different types of Diagnostic Studies and Imaging available that assist physicians and practicing nursing counterparts in diagnosing diseases. There is also the devolopment electronic medical records that was generated to improve patient care. Lastly, nursing regulation is important. Initially there was no regulations and standards. As it gained strength in the workforce, governance of this profession was needed. This is where associations and boards came into existence to help and regulate the profession and generate a code of conduct that nurses are presently able to follow.

One of the most important skills a nurse needs is clinical judgment. This is a skill that can be learned. Reflect on your own ability to critically think and the reading for this module.

When see yourself as a nurse and giving care, discuss what qualities and behaviors you possess that make you a person that would have good clinical judgment. What three strategies can you use to develop better clinical judgment? As a nurse, what areas of the nursing process do you think might be more challenging to you given the critical thinking and clinical judgment skills you have now?

Most of us don’t think about how we think. Thinking is a skill. In the reading critical thinking is defined as “your ability to focus your thinking to get the results you need in various situations.” This means that you are able to use your thinking abilities to clarify goals, examine assumptions, evaluate evidence, accomplish actions, and evaluate decisions or conclusions. And, it is when you are able to do this consciously you are “critically thinking.”

In nursing it is essential that we think critically because the decisions we make influence the care and health outcomes of others. Critical thinking also leads to clinical reasoning and then to making clinical judgments. It is when we are able to make informed and conscious clinical judgments we are able to provide safe quality nursing care.

Critical thinking skills include reflection, interpretation, analysis, inference, and evaluation. Critical thinkers are open-minded, systematic, and have self-confidence. In your reading there are critical thinking indicators. Review these behaviors, qualities, and attitudes to see which you possess.

As described in your reading there area barriers to critical thinking. Just a few are age/maturity, one’s self confidence, past experiences, dislikes, prejudices and biases, one’s anxiety and fears, negativity, motivation. These potential barriers need to be in full awareness as a nurse can the can negatively affect clinical judgment.

When comparing critical thinking, clinical judgment, and clinical reasoning, you will find in your reading that the differences are critical thinking, and clinical reasoning are considered processes and clinical judgment is the result of the process. These are purposeful and end in outcomes.

One way nurses used critical thinking, reasoning and judgment is through the nursing process.

Nursing Process

The nursing process is a clinical reasoning tool. The phases of the nursing process are dynamic and organized steps that lead to clinical decision making. The nursing process includes assessment, analysis and identification of the problem, planning, implementation, and evaluation. When applied to client situations, the nursing process offers a way to organize information to make the best possible clinical decisions. The review of the nursing process in the textbook provides detailed information for each of the phases. You will be learning more about the nursing process in later courses. For now you want to think about it as an organized way of assessing and evaluating information and making informed decisions.

In this module the key emphasis is understanding the differences between critical thinking, clinical reasoning, and clinical judgment and your abilities in each of these areas.
