CASE STUDY
RN John steals used dressing forceps and scissors which are destined for disposal from the hospital in which he works. He donates them to an overseas third world charity. The forceps and scissors benefit many, many people who otherwise would not receive adequate care. RN John does this with the awareness of RN Jane who is both his fiancé and Nurse Unit Manager.
An investigation into the matter is to be undertaken by the Director of Nursing, Ms. Day. Ms. Day approaches RN Jane and says “you either tell me who is doing this or give me $100 to forget about the matter”. RN Jane says she does not know who is stealing but agrees anyway to pay $100 to protect RN John as she believes his actions are honourable.
Jane has that amount in her bank account but cannot get to the bank until tomorrow. Ms. Day will not wait until then. Jane is aware that patient Ian has that amount hidden in his bed locker. She intends to take the money and replace it tomorrow having gone to the bank. On arrival at Ian`s room Jane tells relatives that they should leave and once gone takes the money. Upon taking the money Ian wakes and asks what she is doing. Jane grabs Ian by his injured arm which causes him pain and verbally threatens him not to say anything.
Jane leaves and gives the money to Ms. Day. She uses half the money to buy a bottle of wine and half is donated to charity. Ms. Day lies to hospital management that she has investigated the matter and no one can be identified.
Jane is later charged with stealing $100.
RN John breaks off the engagement with Jane as he does not want to be married to a thief as it will damage his career, even though he has been told the reason for Jane`s actions.

Case Study for Assessment No. 3 (Cholecystectomy)

Trent Black is a 65 year old retiree who is admitted to your ward from the emergency department. On arrival you note that he is trembling and nearly doubled over with severe abdominal pain.

Mr Black indicates he has severe pain in the right upper quadrant of his abdomen that radiate through to his mid back as a deep sharp boring pain. He is more comfortable walking or sitting bent forward rather than lying flat in bed. He admits to having had several similar bouts of abdominal pain in the last month, but ‘none as bad as this’.

He feels nauseated but has not vomited, although he did vomit a week ago with a similar episode. Mr Black experienced an acute onset of pain after eating fish and chips at a fast-food restaurant earlier today.

Admission vital signs are: BP 164/100, HR 132, RR 26, Temp 36, SaO2 96% on 2L of oxygen by nasal cannula.

Mr Blacks abdominal ultrasound demonstrates several retained stones in the common bile duct and a stone-filled gallbladder. He undergoes an ERCP and the stones and bile are released, but imaging reveals that a stone is still retained within the cystic duct, and multiple stones remain within the gallbladder as well. The surgeon decides that Me Black will require a laparoscopic cholecystectomy.

When nurse informaticists are tasked with identifying the most appropriate technology to meet a specific need within a health care setting, there are many questions that must be asked.

Consider the following scenario to address assignment

Riverdale Hospital has come under recent scrutiny for their medication procedures. Many times, paper medication records are not up to date or have been misplaced. As a result, patients have increasingly received their medications at the wrong times. Though each nurse is performing to the best of his or her ability, the fast pace of the hospital has caused some to ineffectively manage patient records.

The lead nurse informaticist, Nancy, has decided that a bar code scanner could help streamline the documentation process while also improving patient quality and safety. Nancy knows that when selecting a bar coding system she must not only examine the hardware and software of the system but also consider the various human factors that can positively and/or negatively affect the outcomes of the system implementation. As such, Nancy asked three of the most reputable bar code vendors to bring sample systems to Riverdale Hospital.

In evaluating each system, Nancy role plays the process of scanning a patient’s bar code. She rolls the coding cart into the room to begin her mock demonstration. First, Nancy scans her identification card to gain access to the medication screen. To scan the patient’s bar code identifier, Nancy then pulls the medication cart to the patient so that the attached scanner reaches the bar code on the patient’s wristband. When the scan is complete, the computer displays a screen that houses the patient’s personal information. By navigating the screens, Nancy finds that she can use the computer to track medication administration. In addition, Nancy is able to view applicable vitals and medication history. As Nancy continues to examine this system, she reflects on the other hardware and software facets she should be sure to consider. She also thinks about how human factors will affect this and other vendor systems.
