II. Nursing Problem Analysis
The purpose of this paper is to provide students with the opportunity to analyze concepts or variables of interest within the context of a problem, in Nursing, using published research studies from peer-reviewed journals. Analysis of health and disease outcomes will be accomplished by examining each research article. Students will analyze each research study for research methods and analyses, in order to better understand the role of the advanced practice nurse in the evaluation of research, in order to seek knowledge for evidence based practice.

The following is a procedure you should follow for this paper.

Prior to our library search class, students should:

1. A. Select two concepts or variables pertaining to a problem. This could be a problem you have encountered in your practice setting, one you have heard about or one that you are interested in, or, a problem based on Healthy People 2020, World Health Organization, or other National/International Health Agendas. Choose a problem in Nursing. Choose a problem which best fits under one of the following themes from the National Institute of Nursing Research.
B. The National Institute of Nursing Research has 4 major themes and 2 cross-over ones for priorities in Nursing Research so you may want to choose a problem which fits under one of them. They include: 1. Symptom Science: Promoting personalized health strategies (some examples are: chronic illness and adverse symptoms such as symptoms resulting in pain, fatigue, disordered sleep or strategies to treat and prevent symptoms of illness across diverse populations and settings; 2.Wellness:promoting health and preventing illness (such as physical, behavioral and environmental causes of illness, behaviors that lead to a healthy lifestyle and choices to develop evidenced-based interventions to promote illness); 3.Self-Management: Improving quality of life for individuals with chronic illness (some examples are: strategies to help individuals with chronic conditions and their caregivers to create a better understanding of these strategies for management of their illness); 4. End of Life and Palliative Care: The science of compassion (some examples are: science to manage the symptoms of life limiting conditions and planning for end-of-life decisions); 5.Promoting Innovation: Technology to improve health (some examples are: science in promoting innovations) and, 6. Innovative Strategies for Research Careers.

One example selected might be: the effects of the use of bright light on patient�s sleep. There are many issues which can be chosen so think about one which best suits your own clinical area of expertise. Think about choosing some problem in patient care.

2. Search the literature by using either key words or concepts (depending on the search engine used) from the problem and locate 10 research, peer-reviewed studies (less than 10 years old- preferably 5 years or less) in Nursing which address these two concepts or variables.

3. Using 5 quantitative, peer-reviewed research articles (out of the 10 selected from the literature search) in Nursing, identify and describe elements of the research process within each of these articles. These elements include:
a. research problem and purpose, or hypotheses if any,
b. literature review,
c. design,
d. sampling strategies,
e. descriptive and/or inferential statistical analyses for the major study concepts or variables (only choose those statistics which fit your major study concepts or variables-do not choose both)
f. summary, conclusions and limitations to the study
(Hint: Be sure to write these steps out, avoiding the use of an outline or exact words from the research article. Avoid the use of a table. Do not take these steps from the abstract but rather select them from the study, using your own words.)

Define the attributes (include antecedents & consequences): As you are looking at the uses and definitions of the concept, make a list of the characteristics you are seeing over and over. You want to try to show the cluster of attributes that are the most frequently associated with the concept and gives the analysis the broadest insight into the concept. Also include the antecedents and consequences. Antecedents are those events or incidents that must occur prior to the occurrence of the concept. Thus an antecedent cannot be a defining attribute. Consequences are those events or incidents that occur as a result of the occurrence of the concept…the outcomes of the concept. In the coping example, an antecedent was an intensely stressful stimulus …the consequence was the regaining of balance. These may be difficult to understand…be sure to refer to the concept analysis examples for further review.

Cases: Write each of these as actual scenarios, you may pull these examples from your own experience, or you may choose to “make them up”.

Model Case: The model case, also often called the exemplar case, is an example of what the concept looks like. You need to apply the concept to a clinical situation sort of like a case study. You can make this up, use an example from your clinical practice, or an example from the literature (It is not appropriate to use the cases developed by an author of a published concept analysis). Bold the defining attributes as your example includes them in the scenario.

Borderline Case: The borderline case is an example that contains some of the defining attributes but not all of them. The case is inconsistent in some way from the concept under consideration. In Chinn & Kramer (2008), they speak of an analysis of fatigue. For the borderline case, the case could include military fatigue clothing. Another example is the concept of mothering. A borderline case might be about a computer motherboard.Related Case: A related case does not contain the critical attributes of your chosen concept, but may contain similar ideas. For example, the concept quality of life may include a related case that is an example of satisfaction with life, well-being or health status. Remember, this should be a demonstration of a situation that is very similar to your chosen concept, but not exactly the same. It will not contain the defining attributes of your chosen concept.

Contrary Case : This case is probably one of the easiest to understand. It is what the case absolutely is not. For example, with the concept of coping, a contrary case would be the host who burns the roast, has a group of people to their house, serves them no food and sends them home. There is no coping to the stressful event.

You will include an introduction & conclusion in the paper as well. Remember that the introduction is really not a long paragraph. Open an introduction with an attention grabber, add a few more overview sentences and close the opening paragraph with the thesis statement. For example, “This paper will include an analysis….”

The conclusion paragraph does not contain any new information or new references; it is a restatement of the main points of the paper.

John is a successful lawyer in a large city firm and Mark is an aspiring writer who has yet to find a publisher for his first novel. They live in John’s three bedroom townhouse at 33, Acorn Avenue, Langley, RX23 4SR, and Mark takes care of the house and cooks all their meals.

At a recent party, John met Ian, with whom he has fallen in love, and they plan to marry. When John told Mark of his plans, he told Mark that he, Mark, would have to leave the house and find another home.

Mark has nowhere else to live and has very limited funds. He refused to leave the house and since then John has made life there very unpleasant for him. He has frequently criticised Mark’s cooking skills and his inability to find a publisher in front of their friends and last week he threw a bowl of pasta at Mark, narrowly missing Mark but breaking a kitchen window.

Last night John came home late, drunk. He took a carving knife out of a drawer in the kitchen, held it against Mark’s neck and told him that if he had not moved out by the next day, he, John, would not be responsible for his actions.

Mark is terrified of what John might do but he wants to continue to live in the house because of his circumstances and as it is close to the hospice where his terminally ill father is living.

Case Study

In 2009, Sirius Pink purchased a large, disused school for £300,000. He now intends to convert the property into a nursing home, which he will then manage. Sirius will need substantial capital in order to pursue his plans, and his cousins, Petunia White and Dumbledore Old, have indicated that they are willing to join the business and help him run the nursing home. Hermione Yellow, a retired housewife, is also willing to invest £100,000 in non – voting preference shares in the company. Hermione is insisting that she be given the right to remain a director for as long as she is a shareholder, so that she can `keep an eye on her investment’.

The following additional information has been provided:

– A shelf company, JLIT Shelf Company 06897 Limited (fictitious) will be used as the vehicle for the new business.

– The company’s name will be changed to Rest Care Ltd.

– The company will have an issued share capital of £150,000, comprising 150,000 fully paid ordinary shares of £1 each.

– Sirius, Petunia and Dumbledore will all invest £50,000 in exchange for 50,000 ordinary shares at £1 each.

– The company will adopt the Model Articles for private companies limited by shares.

– Sirius, Petunia and Hermione will be appointed directors of the company.

– Once established, the company will enter into a loan agreement with Grainger Bank for £70,000 to be secured by way of a first

Your task is to undertake the following TWO (2) exercises, producing the relevant documentation where appropriate:

(A)

(B)
You are also required to advise Sirius with regard to the following issues:

i) The tax implications for the company and Sirius in respect of any salary or dividends received by him from the company.

ii) Steps to protect him and Hermione from removal as a director.

Assuming that Sirius intends to sell the disused school which he bought in 2009 to the company for £500,000, explain how the law governing promoters would apply to such a sale.

(D)
Advise Sirius on steps the company needs to take to accept the proposed loan from Grainger Bank plc and draft the necessary documents to secure the loan.
